Referral
Letter/Disability Form
Disability forms provided by your
employer will be completed after office hours. The patient must complete
his/her section of the form and sign the release. The first form is
completed as a courtesy to our patients; every form thereafter must be
accompanied by a nominal fee. In most cases, forms will be ready for
pick-up the next day. Should you require a form to be faxed, there will be
an additional charge.
