Policies and Procedures
Appointments
We see all patients on an
appointment basis and you must be referred to our office by your primary
care provider. We try and see all patients on time and request you extend
the same courtesy to us. If you cannot keep an appointment, please call
our office. Try to give 24 hours notice so this time can be reassigned to
another patient.
In order to assist the physician
with your treatment, it is necessary that you bring all previous x-rays,
laboratory results or past medical records. If you are being seen for any
lower extremity problem, you are welcome to bring a pair of shorts to wear
for you convenience and comfort.
PATIENTS UNDER THE AGE OF 18 YEARS WILL NOT BE SEEN
WITHOUT A PARENT OR GUARDIAN IN ATTENDANCE.
